Health Questionnaire 

As an employee of EK Recruitment Pty Ltd (ABN: 95 620 229 763), our foremost concern is your health and safety. This questionnaire is designed to assist us in ensuring that our employees are not placed in an environment or given tasks that would likely result in physical or mental harm.

 

You are required to disclose to EK Recruitment Pty Ltd any or all existing or pre-existing injuries, illnesses or diseases suffered by you which could be accelerated, aggravated, exacerbated, deteriorate or recur by you performing the responsibilities associated with the employment for which you are applying with EK Recruitment.

 

If you fail to disclose this information or if you provide false and misleading information in relation to any pre-existing injury/condition, you and your dependents may not be entitled to any form of workers’ compensation and this may also constitute grounds for disciplinary action or dismissal.

 

IMPORTANT: The information obtained in this questionnaire will be treated in strict confidence and will only be used in conjunction with the requirements of a client assignment.

Drug & Alcohol Testing

I hereby agree, upon request made under the drug/alcohol testing policy of EK Recruitment Pty Ltd to submit to a drug and/or alcohol test and to provide a sample of my urine, breath, saliva and/or blood for analysis. I understand and agree that if I at any time refuse to submit to a drug or alcohol test under company policy, or if I otherwise fail to cooperate with the testing procedures, I will be subject to immediate termination. I further authorizing and give full permission to have the Company and/or its company physician send the specimens collected to a laboratory for a screening test for the presence of any prohibitive substances under the policy, and for the laboratory or other testing facility to release any and all documentation relating to such test to the Company and/or any governmental entities involved in legal proceeding and investigation connected with the test. Finally, I authorize the Company to disclose any documentation relating to such test to any governmental entity involved in a legal proceeding or investigation connected with the test.

 

I understand that only duty-authorized Company officers, employees, and agents will have access to information provided or obtained in connection with the test; that they will maintain and protect the confidentiality of such information to the greatest extend possible; and that they will share such information only to the extent necessary to make employment decisions and to respond to inquiries or notices from government entities.

 

I will hold harmless the Company, its company physician and any testing laboratory the Company might use, meaning that I will not sue or hold responsible such parties for any alleged harm to me that might result from such testing, including loss of employment or any other kind of adverse job action that might arise as a result of the drug and alcohol test even if a Company or laboratory representative makes an error in the administration or analysis of the test for the reporting of the results. I will further hold harmless the Company, its company physician and any testing laboratory the Company might use for any alleged harm to me that might result from the release or use of information or documentation relating to the drug or alcohol test, as long as the release or use of the information is within the scope of this policy and the procedures as explained in the paragraph above.

 

This policy and authorization are in a language I understand and I know that any questions about the test or the policy can and will be answered when I meet with the Company.

 

I understand that during work hours the company will require a drug and alcohol test should I be involved in an incident or injury.

 

EK Recruitment or their client can at any time conduct a random drug and alcohol test.

 

Please note: You will be required to sign a form in person which confirms that you have read and consent to the alcohol and/or drug testing terms and conditions with a witness present.

Timesheet Information

It is the responsibility of the Casual/Contractor (You) to get the timesheet signed.

 

No timesheet will be processed without a signature. 

Timesheets for the week must be submitted by Sunday night at 11:59pm. Payment will be made the following week into the account nominated by the Casual/Contractor. There will be no advance payments, so please don’t ask.

Please note that if you do not submit your timesheet on time, your pay will be delayed. 

Meal Breaks

EK Recruitment Pty Ltd would like to remind all employees of their obligations in relation to meal breaks.

 

Please ensure that you take your meal break in accordance with the direction from EK Recruitment.

 

It is your responsibility to notify EK Recruitment if you are unable to take your meal break. Only in exceptional circumstances will EK Recruitment allow you to work through your lunch break. Employees are required to seek EK Recruitment’s approval in these circumstances.

 

Failure to comply with the above will be considered misconduct and may lead to disciplinary action.

National Criminal Background Check Payment Agreement

A condition of working on assignments through EK Recruitment, is that some of our clients require casuals place onto their site, to have a National Criminal Background Check conducted. This check covers off on any criminal history that has occurred under your name.

If you are placed at a site that requires you to have a National Criminal Background Check conducted, an EK Recruitment Representative will contact you to let you know that a check is being lodged and a $50 deduction will be processed from your next scheduled pay.
